Frequency plus severity across four symptoms, scored out of 28.
How it is calculated
- Four symptoms are rated twice: how often they occur, from 0 (never) to 3 (constant), and how bad they are, from 0 (no problems) to 4 (intolerable).
- The score is the plain sum of all eight ratings, so it runs from 0 to 28.
- Because it is a sum rather than a mean, a partly completed questionnaire understates the score and is not comparable with a complete one.
- A total of 0–6 is normal, 7–15 mild to moderate, and 16–28 severe. A score of 6 or more is the commonly used flag for dry eye disease.
- Unlike the OSDI, SPEED asks about symptoms over the preceding three months as well as today, which makes it more sensitive to a fluctuating course.
